Does Medicare Pay for Home Care in North Carolina?
Many Eastern NC families ask if Medicare covers in-home care. We explain what Medicare's home health benefit includes and what it doesn't, so you can plan clearly.

If you're helping an aging parent in Eastern North Carolina, one of the first questions you likely have is about cost: 'Will Medicare pay for home care?' It's a common question we hear from families from Greenville to Fayetteville. The short answer is complex: Medicare covers some specific types of in-home care, but it does not cover the long-term personal assistance many families seek. Understanding this distinction is the first step toward making a clear, confident plan for your loved one's well-being right here at home.
First, it’s helpful to see Medicare for what it is: a federal health insurance program, not a long-term care solution. Its primary purpose is to cover medical costs for older adults and certain younger people with disabilities. When it comes to home care, Medicare focuses on short-term, intermittent care that is medically necessary for recovery from an illness, injury, or surgery. It’s designed to help someone get better and regain their independence, not to provide ongoing daily support for chronic conditions or aging in place.
Under specific conditions, Medicare Part A and/or Part B will pay for what's called the 'Home Health Benefit.' This benefit covers part-time skilled nursing care ordered by a doctor; physical therapy; occupational therapy; and speech-language pathology services. It might also cover a home health aide if you are also receiving skilled care. The key word here is 'skilled.' This care must be administered by a licensed professional to manage a medical need, like changing wound dressings or managing IV medication, not just helping with daily routines.
This brings us to what Medicare almost never covers: custodial or personal care. This is the type of non-medical help that most families are looking for. It includes assistance with Activities of Daily Living (ADLs) such as bathing, dressing, eating, using the toilet, and moving around the house. Medicare does not pay for 24-hour-a-day care at home; meal delivery services; or homemaker services like shopping, cleaning, and laundry. If personal care is the only type of care your parent needs, Medicare will not cover the cost.
To qualify for the Home Health Benefit, two main criteria must be met. First, a doctor must certify that your parent is 'homebound.' This means it is a major and taxing effort for them to leave the house, and any trips out are infrequent and for short durations, like a medical appointment or a religious service. Second, the doctor must certify the need for 'intermittent skilled care.' This means the care is part-time and provided by a skilled professional. Both conditions must be met for Medicare to approve coverage.

When families in cities like New Bern, Kinston, or Rocky Mount realize that Medicare won't cover their parent's need for daily assistance, they often turn to private-pay options. This means paying for care out-of-pocket or using a long-term care insurance policy. While it represents an expense, it also provides tremendous flexibility and choice. You can hire a caregiver for as many or as few hours as you need to provide companionship, help with meals, ensure medication reminders, and assist with personal grooming, allowing your loved one to live safely and comfortably at home.
Navigating this path can feel overwhelming, but you don't have to do it alone. Once you’ve determined that private-pay personal care is the right direction for your family, we can simplify your next step. Instead of making dozens of calls, you can fill out one confidential request with us. We then connect you with a carefully selected, pre-screened home care provider serving your town in Eastern North Carolina. Let us help you find a potential match so you can focus on your family.
